Only this form or the mail-in form available from the IAC may be used for nominations. No other materials should be submitted. All valid nominations not selected for a 2007 award will be eligible for future consideration. Submissions are due by 5:00 p.m. November 1, 2006. Nominees must be willing and available to attend the awards ceremony. Recipients should agree to assist with media requests related to their award. If an individual is being nominated posthumously, provide name and contact information for a relative who will be able to accept the award.
